The time has come to fill out your CSD Boosters Membership Form! ​You can register your family in two easy steps!​

STEP 1 :  REGISTER

Please send an email to csdhotfeet@gmail.com and include the name of your dancer(s) as well as a good contact number to reach you!

STEP 2 :  SEND PAYMENT

​To pay your membership fee of $20, you may use on of the following methods:
